In making this application to the Nebraska One Box Pheasant Hunt, I understand that I will be obligating myself financially for all fees that are required for the hunt. Also, that all my prepaid fees are forfeited if I do not submit a written cancellation thirty (30) days prior to the hunt. 

By checking this box, the applicant consents to the use of his or her name and likeness for promotional purposes and dissemination of news information and press releases for the use and benefit of the Nebraska One Box Pheasant Hunt, Inc. and its related organizations
